How to Apply for the NDIS: A Simple Checklist

Applying for the NDIS (National Disability Insurance Scheme) can feel overwhelming at first, but we’ve found a helpful checklist to make the process easier. This checklist guides you step by step so you can be confident you’ve got the right information ready before you apply.

Step 1: Check if you’re eligible

Before applying, make sure you meet the basic requirements. You may be eligible if you:

  • Are under 65 years old

  • Live in Australia and are an Australian citizen, permanent resident, or hold a Protected Special Category visa

  • Have a permanent or significant disability that impacts your everyday life

Step 2: Get your information together

This checklist reminds you to collect documents that support your application. This usually includes:

  • Proof of age and residency (e.g. passport, Medicare card)

  • Information about your disability (medical reports, assessments, or letters from health professionals)

  • Evidence of how your disability impacts your daily activities

Step 3: Apply to the NDIS

  • Call the NDIS on 1800 800 110 and ask for an Access Request Form

  • Visit a Local Area Coordinator (LAC) or NDIS office for support

  • Download the Access Request Form from the NDIS website
